Job Description

Seeking a dedicated Nurse Practitioner or Physician Assistant to join our Providence Primary Care Team. In this role, you will work closely with advanced practice clinicians in a team-based care model that also includes social workers, nurse case management and clinical pharmacists. Behavioral health is embedded in the clinic as well, with a Psychiatrist and onsite Pharmacy available to patients and to provide support for Primary Care teams. • One to three years of experience in outpatient Family Medicine is preferred.
• Ideal candidate will be flexible and eager to work as a part of a team in a patient-centered medical home
• Current patient panel is pediatrics and up
• Monday through Friday Clinic schedule
